Test prints of two different objects: 20 mm cylinders with a 2 mm wall thickness, and 6 mm cylinders.
On closer inspection, the "threads" are the inside perimeter not sticking.

Any suggestions on the best approaches to debugging printing problems?

Would you upload your slic3r settings? I can have a look at those.
Then again, it may be a problem in the real world. Try printing one of these (http://www.thingiverse.com/thing:16799) and see how it works.

The translucent blue filament is not camera friendly!

Issue that I can see: strings between the blocks, column deformation, & the bottom couple of layers are a bit wider.

Looking over your settings, I have a few suggestions.
Fill density: Slic3r take percents, unlike skeinforge, which takes decimals. i.e. 20 is 20% fill, 0.2 is 0.2% fill, etc.
Temperature: Extrude at the lowest temperature possible. For those speeds, you might be able to go as low as 165. If you're running marlin, you'll need to modify the minimum extrusion temperature. Experiment to find the lowest temperature you can reliably extrude at, and use that.

Your first layers stick out because you have the z-axis set too low. My guess is that you're doing this to make the first layer stick better. Wat you can do instead is change the first layer height ratio to 0.6, or something like that. Then set your zero to when the nozzle is just touching the platform.
Give it another shot. I'll be interested to see what that changes.

Just upgraded the Z axis to ACME lead screws and added supports for the top of the rods.

Cooling

A couple of picture of my current cooling setup. I tried a duct that surrounded the extruder, but wasn't happy with the loss of visibility.
I ended up going with a Ducted cooling fan and adapter ring.
The duct is modified with an additional port for cooling the back of the extruder, and shields to direct the airflow downward.
