Looking over your settings, I have a few suggestions.
Fill density: Slic3r take percents, unlike skeinforge, which takes decimals. i.e. 20 is 20% fill, 0.2 is 0.2% fill, etc.
Temperature: Extrude at the lowest temperature possible. For those speeds, you might be able to go as low as 165. If you're running marlin, you'll need to modify the minimum extrusion temperature. Experiment to find the lowest temperature you can reliably extrude at, and use that.
Your first layers stick out because you have the z-axis set too low. My guess is that you're doing this to make the first layer stick better. Wat you can do instead is change the first layer height ratio to 0.6, or something like that. Then set your zero to when the nozzle is just touching the platform.
Give it another shot. I'll be interested to see what that changes.
